Keflex 500mg is the beta-lactam antibiotics, which comes under the category of first generation cephalosporins. It is a tablet dosage form and it is prescribed in the treatment of a wide variety of bacterial infections, which include respiratory tract, skin, middle ear, bone and urinary tract infections. Keflex 500mg got its FDA approval in 1971. Keflex 500mg is only effective for bacterial infections and it cannot be used to treat viral infections.

Mechanism of action

Keflex 500mg kill the bacteria by disrupting the cell wall of bacteria. Peptidoglycan layer plays a major role in the formation of cell wall of bacteria which is ruptured by the Keflex 500mg.


Keflex 500mg should not be used in patients with the following condition:

  • Kidney diseases
  • Large intestine inflammation
  • Clostridium Difficile bacteria related colitis
  • Cephalosporins beta lactam hypersensitivity.

Drug interaction

  • When metformin is administered together with Keflex 500mg, dose of metformin must be readjusted, so as to decrease the toxicity.
  • Co-administration of Keflex 500mg and probenecid should be avoided.
  • Keflex 500mg should not be used with warfarin as it may increase the risk of bleeding.


  • Store this drug at normal room temperature in a well closed container.
  • Keep it out of reach of children.

Doses of Keflex 500mg

Regular adult dose for:

  • Bacterial endocarditis prophylaxis- 2 gram as a single dose before main course.
  • Cystitis- 500 mg twice a day for 7-14 days.
  • Otitis media- 500 mg four times a day for 10-14 days.
  • Pharyngitis- 500 mg twice a day.
  • Skin infection- 500 mg twice a day.
  • Osteomyelitis- 500 mg four times a day for 4-6 weeks.
  • Pyelonephritis- 500 mg orally four times a day for 14 days.
  • Upper respiratory tract infection- 250-500 mg four times a day for 7 to 10 days.
  • Bacterial infection- 250-500 mg four times a day.

Regular pediatric dose for:

  • Pharyngitis- 12.5 to 25mg/kg twice a day
  • Skin and soft tissue infection- 12.5 to 25 mg/ kg twice a day
  • Bacterial endocarditis prophylaxis- 50 mg/kg as a single dose before starting main course.

Missed dose

In case if your dose is missed, take that dose as soon as you remember, but if it is time for the next dose then skip that dose. Do not double your dose in case of a missed dose, as it may expose you to the adverse effects of this drug.

Over dose

If a higher dose than the recommended dose is ingested following symptoms may occur:

  • Nausea
  • Vomiting
  • Diarrhea
  • Epigastric distress
  • Hematuria

Side effects

Most common side effect associated with the use of Keflex 500mg is diarrhea.

Following side effects rarely occur while using the Keflex 500mg

  • Abdominal pain
  • chills
  • Blistering or loosening of skin
  • Cough
  • Dizziness
  • Fever
  • Anorexia
  • Light coloured stool
  • Rashes


  • Do not use this drug without prescription as it may increase the risk of antibiotic- resistance that leads to decrease in efficiency of the drug.
  • Stop taking medication if allergic reactions occur to patients.
  • Dose of the Keflex 500mg must be revised carefully in case of patients with renal impairment.
  • In case of pregnancy and feeding mothers use Keflex 500mg only if clearly needed.
  • Keflex 500mg has the ability of lowering blood clot, so consult your doctor if you have unusual bleeding or bruising.
